Academic Petition - Undecided College/Major, Undergraduate Studies College, & Non-Degree Seeking Students Only

  This form is to request a late withdrawal from one or more courses after the last day to withdraw from coursework and/or request to waive a University requirement

  This link must be sent via email to each instructor of the course(s) you’re requesting a late withdrawal from along with a description of your request, name, Z#, course (i.e. - ENC 1101), and semester

Instructor Documentation Form

StudentsÌý (Undecided College/Major, Undergraduate Studies College, & Non-Degree Seeking Only) Ìýwho desire to petition for late or total withdrawal from a course(s) must have an Instructor Documentation Form submitted for each course in which late withdrawal is sought.

The instructor of record must use their Â鶹¾«Æ·ÊÓƵemail address to log in and fill out the form. Once it is entered, you will be taken to the Â鶹¾«Æ·ÊÓƵsingle sign in to complete the form. The form can be accessed by clickingÌý Ìý.

Reinstatement

• Undecided College/Major, Undergraduate Studies College, & Non-Degree Seeking Students Only
  This form is to request reinstatement to Â鶹¾«Æ·ÊÓƵafter having been previously academically dismissed for 3 semesters
  

Part-time Requests

• Ìý(must use Â鶹¾«Æ·ÊÓƵemail to access)
  Request to attend as a part-time student for students with up to 45 earned credits (above 45 credits please contact your College advising office)

Gen Ed/DARS Request

• Ìý(must use Â鶹¾«Æ·ÊÓƵemail to access)Use this form to request possible course substitution

Gen Ed Degree Audit Request

  The purpose of the Student Gen Ed Audit is to assess how and if a students’ credits can be used to fulfill Gen Ed degree requirements

  First and Second Year Students ONLY (45 or fewer completed credits and Undeclared Majors with any number of completed credits)
